How Insurance Coverage for Treatment Works

Deductible

The amount you must pay out of pocket before your insurance starts paying for treatment. If you have already met your deductible for the year, your insurance coverage will likely start immediately.

Your share of the cost of treatment after the deductible has been met, either as a flat fee or a percentage of the total cost of treatment.

The most you will pay during a plan year. Once your total out-of-pocket expenses reach this limit, your insurance will cover 100% of covered services for the remainder of the plan year.
